Contact Information and Referrals

The Memory Disorders Clinic welcomes physician inquiries, consultations and referrals.
For general information or to schedule an appointment, please call (614) 293-8531, Monday through Friday, 8:30 am - 4:30 pm. Prior to the initial appointment, patients will receive information explaining the Memory Disorders Clinic and the appointment process. A map will be provided to assist in locating the Memory Disorders Clinic. Patients are requested to bring old medical records, the actual scans or x-rays from previous evaluations and a list of all their medications.
Our staff will maintain regular and close communication with the referring physician throughout the evaluation and treatment process. Written summaries of the patient's condition, progress and treatment plan will be sent to the referring physician. Patients will return to their family or referring physician for their primary care.

Location

The Memory Disorders Clinic is located on the first floor of the Clinic Building at The Ohio State University in Columbus, Ohio. Patients may park in the visitor-parking garage "H" or valet parking is available in front of the Clinic Building on West 10th Avenue.
